Sending Specimens

Setting up your account

To send specimens, please download and print the New Client Registration Form and email it to us at info@monarchhistology.com. We will then send you a the necessary supplies and forms to be used for your first shipment.

Submitting specimens

All supplies, submission forms needed to submit your specimens are provided by Monarch Histology Services. In-house courier service is available for those clients within our courier network area (please call for availability (352) 262-9163 (ask for Steve). Specimen cassettes should be submitted in zip-lock plastic bags wrap with tissue cassettes lined in formalin soaked paper towels with the submission form in a separate zip-lock bag inside the main bag to protect it from leaks in transit. For clients who plan to send specimens on a regular basis. A regular shipping address, contact information and your payment information is to be set up for convenience of a safe and speedy turnaround.

Billing

All accounts are billed during the first week of the month for the preceding month’s charges.
